Our Case Management is at the Center of Success
Case Management aims to serve as the central point of contact for all facets of the total clinical and family recovery team.
Through case management, My Interventionist works with all treatment providers to coordinate care for your loved one and ensures that the recovery team is kept informed.
Case Management provides an easy-to-follow recovery plan orchestrated with My Interventionist experience. When changes are made to any one of the spokes of the treatment wheel, My Interventionist will update all providers and all members of the recovery team to keep things current.
My Interventionist case management services start as soon as the engagement begins and continue for a minimum of one year. Individual client coaching sessions are setup weekly. Recovery team sessions are setup monthly. Additional individual and group sessions can be scheduled as needed for an additional fee.
